3. Kumasi campus at St. Anne’s Anglican Church premises, Ashanti New Town (Ash. Town), near the Manhyia Palace, Kumasi.
B. Fill out the form
Applicants must fill All REQUIRED fields with correct information. Note that, improper filling of the application form may terminate your application.
C. Attach Documents
Attach the following documents:
1. Please include photocopies of certificates and other relevant documents.
2. Mature applicants should attach a copy of their birth certificate.
3. Completed applications should be sent (together with a payment order/banker’s draft of GH₵ 100.00) via post to the address on page 4 of this form.
4. Attach two passport-size Photographs to the form.
